Medical Release 2024
The Medical Release Form is good for a full calendar year regardless of the events a student will participate in. What makes the Medical Release apply to specific events is a custom Event Release Form.
Look for these forms in the youth room with details about each event as they arise.
Scholarship Form
At MFBC, there is no reason why finances should ever keep a student from fully participating in a youth ministry event. Donations have already been given and continue to be contributed toward our scholarship funds. If for any reason funds are needed to participate in an event, fill out and return this form. Scholarships are decided on by the Youth Committee and awarded directly to the student's activity account for payment.
Broadway Grand Prix Form
All youth, guests, and families are invited. Parents will provide their own supervision for
younger children and will not be counted as a chaperone for youth. Payment and forms due before
we leave. Activity accounts may be used if funds are available.
Top Golf Form
